Se ha celebrado en Castellón el XL Congreso de las Enfermedades Cardiovasculares organizado por la Sociedad Valenciana de Cardiología, y he tenido el placer de ser ponente en una mesa científica muy interesante sobre un tema que me apasiona, DEPORTE Y ARRITMIAS, con el título de «ENTRENANDO CON RITMO».
Con la moderación de la Dra. Izquierdo (Hospital Universitario y Politécnico La Fe de Valencia) y el Dr. Bertomeu (Hospital HCB Benidorm), expuse mi sesión sobre: «Deporte y fibrilación auricular. ¿cómo, cuándo y a quién?».
A modo de resumen, a lo largo de mi exposicion remarqué los grandes beneficios que tiene la actividad física sobre la salud, con reducciones del 35% en el riesgo de muerte cardiovascular (CV) y del 33% en mortalidad por cualquier causa. Y estos beneficios no sólo son sobre la esfera CV, si no también, sobre problemas neurológicos, endocrinos, osteomusculares y oncológicos. La prescripción de actividad física debería ser una norma habitual desde la consulta de atención primaria y especializada, pocas medicinas aportan resultados tan espectaculares y tan globales.
Y por otra parte está la fibrilación auricular (FA), verdadera pandemia CV de este siglo XXI. Así se comprueba que el grupo de personas con más de 55 años con numerosos factores de riesgo CV (HTA, Diabetes, Tabaquismo, hipercolesterolemia, Enf. Renal Crónica…) el riesgo de FA aumenta considerablemente (1 de cada 2-3 personas tendrán esta arritmia).
¿Qué papel juega el ejercicio físico sobre la FA?:
1/ Existe una relación compleja entre la actividad física y la FA. El ejercicio de intensidad moderada se considera factor protector de esta arritmia. Mientras que el ejercicio de intensidad muy elevada y la inactividad pueden favorecer el desarrollo de FA, apreciándose el patrón en forma de U.
2/ Hay unos mecanismos fisiopatológicos de FA en el deportista de resistencia con alta evidencia, como son: mayor fibrosis auricular, inflamación incrementada, dilatación auricular, incremento del tono vagal.
3/ En deportistas jóvenes de competición, la prevalencia de FA es muy baja (0.3%).
4/ Se remarca un concepto emergente: el corazón del atleta puede ser proarrítmico. Y aquí podemos hablar del Síndrome PAFIYAMA: FA paroxística, varones, ≤60 años, práctica prolongada de ejercicio de resistencia vigoroso, fracción de eyección (FEVI) preservada.
5/ Los ciclistas de resistencia y los nadadores: mayor riesgo de desarrollar FA.
6/ Es esencial realizar intervenciones dirigidas al estilo de vida y modificación de los diferentes factores de riesgo en el seguimiento de estos sujetos.
A modo de conclusión: Hay que moverse, poco es mejor que nada, ya no valen excusas.
