Hago mención en este post del primer Documento de Posición en español, publicado por la Sociedad Argentina de Lípidos, sobre la LIPOPROTEÍNA-A, a partir de ahora Lp(a).
Os resumo lo más importante de este excepcional documento por su claridad e información, y os dejo el enlace al artículo al final.
¿Qué es la Lp(a)?
Se trata de una partícula similar a la lipoproteína de baja densidad (LDL) o colesterol-malo, que está unida a la apolipoproteína (Apo[a]). Su valor se determina de forma genética y los valores de Lp(a) en sangre persisten estables a lo largo de la vida del individuo, sin verse afectados de manera significativa por los hábitos de alimentación, actividad física, inflamación crónica (es un reactante de fase aguda) o el envejecimiento, es decir, su valor suele ser CONSTANTE de por vida. Aproximadamente entre el 20-30% de la población mundial tiene elevadas las concentraciones de Lp(a).
¿Cuántas veces tengo que analizarme la Lp(a)?
Como los niveles plasmáticos de Lp(a) están determinados genéticamente, podemos indicar que una medición de Lp(a) en la vida es suficiente para determinar el riesgo cardiovascular de los pacientes. Pero hay situaciones en las que estas concentraciones pueden variar (pacientes con enfermedad renal crónica, cirrosis hepática y hepatitis viral, hipotiroidismo no controlado, mujeres posmenopáusicas que reciben terapia de reemplazo hormonal o ante el inicio de tratamiento para disminuir los niveles de Lp(a) y evaluar la respuesta al tratamiento).
¿Qué nos indica tener una Lp(a) elevada?
Se trata de un ser un factor de riesgo para padecer enfermedades cardiovasculares, en concreto, infarto de miocardio, accidente cerebrovascular (ACV) y estenosis valvular aórtica. Así, los estudios han mostrado que el riesgo de infarto de miocardio es un 75% mayor cuando los niveles de Lp(a) son superiores a 30 mg/dl, que es el punto de corte a partir del cual podemos decir que los niveles están elevados. En la estenosis valvular aórtica, el riesgo es significativamente superior con niveles de Lp(a) > 40-60 mg/dl.
¿Por que la Lp(a) está implicada y relacionada con las enfermedades cardiovasculares?
Con los estudios actuales, podemos indicar que el efecto es multifactorial, por una parte interviene en procesos inflamatorios asi como en fenómenos protrombóticos.
¿Hay algún tratamiento eficaz para disminuir los niveles de Lp(a) en sangre?
Desgraciadamente no existe una terapia farmacológica específica para provocar su disminución. Incluso las estatinas podrían aumentar en un 10-20% los valores de Lp(a); el resto de fármacos para bajar el colesterol (ezetimibe, fibratos) tienen un efecto neutro. Los inhibidores de la PCSK9 consiguen un descenso que podría llegar al 25%, y esta disminución puede ser mayor en aquellas personas con valores > 120 nmol/l, aunque no disponemos de estudios completos para ver su efecto significativo. Podemos subrayar que estamos en una fase de ausencia de tratamiento para conseguir una reducción eficaz y con resultados positivos a nivel de disminuir eventos cardiovasculares.
Así pues, nos encontramos ante un nuevo factor de riesgo cardiovascular (como puede ser la hipertensión arterial, el tabaquismo, la diabetes, el colesterol, el sedentarismo,…) y seguro que pocos años tendremos más e interesante información sobre la Lp(a).
