Las estatinas son uno de los grupos farmacológicos que más se emplean en la actualidad, disminuyen los niveles del conocido “colesterol malo” (LDL) y numerosos estudios han demostrado su importante papel sobre la prevención de eventos cardiovasculares. Pero también es conocido que existe una tendencia social muy mediática (un “lobby”) en contra de su uso por los efectos secundarios que pueden ocasionar (miopatías, problemas hepáticos) que quieren inclinar la balanza en contra de su empleo.
Como decía, las estatinas se recetan a millones de personas en todo el mundo, y la simvastatina (una de las más antiguas y más económica) sigue constituyendo alrededor del 40% de todas las recetas de estatinas en el Reino Unido en 2019 y es la segunda estatina más comúnmente recetada en los EE. UU. Personalmente, me inclino hacia el uso de estatinas más nuevas y con menos efectos secundarios.
La terapia con estatinas puede causar miopatía en un porcentaje pequeño (dolor muscular o debilidad asociada con niveles marcadamente elevados de creatin-quinasa (CK). Es usual que los pacientes que reciben una estatina puedan describir dolor o debilidad muscular sin niveles elevados de CK en sangre, pero esto también se aprecia en pacientes que reciben un placebo similar, lo que sugiere que estos “dolores” no son causados farmacológicamente por la estatina (es decir, son un efecto nocebo). Diferentes estudios han valorado la incidencia de miopatía en aprox. 1 por 10.000 personas-año con regímenes estándar de estatinas (como simvastatina 40 mg diarios), pero también es cierto que estos pacientes están “polimedicados” y las interferencias con otros medicamentos es conocida. Así, el verapamilo se asoció con un riesgo ocho veces mayor; niacina-laropiprant (muy usado en pacientes chinos, que tienen un riesgo 10 veces superior a los europeos a padecer mialgias por estatinas, y que fue retirado por la Agencia Europea del Medicamento en 2013), diltiazem multiplicó más del triple estos riesgos; y los betabloqueantes y diuréticos con alrededor de 65 a 75% más de riesgo.
Por eso es importante resaltar este trabajo publicado en European Heart Journal, que proporciona datos de tres ensayos grandes (HPS, SEARCH y HPS2-THRIVE) que informaron sobre 58.390 individuos tratados con simvastatina durante una media de 3,4 años, donde examinaron la asociación entre los factores de riesgo para la miopatía relacionada con la simvastatina y riesgo de otros síntomas musculares.
El riesgo absoluto de miopatía (dolor muscular inexplicable o debilidad con CK> 10 del valor estándar de laboratorio) debido a los regímenes estándar de estatinas es bajo (171 casos: 0.3%), pero se puede identificar a las personas que tienen un riesgo elevado combinando una serie de factores de riesgo. El nuevo dolor o debilidad muscular inexplicable que no se asoció con un diagnóstico de «miopatía» (dentro de los 28 días) y que se definió como «otros síntomas musculares», se produjo en 15.208 individuos (26%). La CK debe medirse en pacientes que informan síntomas musculares y controlarse en la pequeña minoría que tiene niveles de CK moderadamente elevados, y en el caso de normalidad, debe alentarse a que continúen tomando su terapia con estatinas.
El análisis revela factores de riesgo independientes para “miopatía” con dosis más altas de simvastatina: etnia china, sexo femenino, mayor edad, índice de masa corporal (IMC) más bajo, diabetes mellitus tratada con agentes hipoglucemiantes y uso concomitante otros fármacos, como son niacina-laropiprant (ya retirado), verapamilo, betabloqueantes, diltiazem y diuréticos. Los autores informan que la «miopatía» en los varones europeos solo ocurrió en diabéticos con un IMC <25 que se tratan con un betabloqueante y diuréticos. En las mujeres caucásicas, la «miopatía» se limitó a un IMC de 20.
¿Qué debemos aconsejar en caso de aparición de dolores musculares?
- Excluir otras causas de síntomas e interacciones musculares, informando detalladamente al paciente sobre la reducción de riesgos a largo plazo y la seguridad de estos medicamentos, y las razones por las que está tomándolas.
- Marcar 3-4 semanas de descanso de estatinas.
- Ante la resolución de síntomas: deberíamos reiniciar esta medicación, pero usando una estatina diferente, comenzando con dosis muy baja, aumentando lentamente la dosis, por ejemplo cada 2 semanas, hasta obtener la dosis de estatina tolerable más alta.
- Deberemos solicitar un nuevo control analítico para conocer como están los niveles del «colesterol malo» (LDL-Cho).
- Si no se alcanzan los objetivos adecuados según nivel de riesgo del paciente, deberíamos combinar la dosis más alta tolerada de estatina con ezetimibe o con inhibidores de PCSK9.
- Sin resolución de los síntomas: Buscar otras causas.
En definitiva, estos datos demuestran claramente el riesgo absoluto extremadamente bajo de «miopatía» relacionada con la simvastatina y proporcionan criterios para identificar a las personas con un riesgo alto y bajo.
