Se ha aprobado una dosis de 0,5 mg diarios de colchicina para su uso en pacientes de alto riesgo que tienen riesgo inflamatorio, con el fin de reducir eventos CV en pacientes de alto riesgo (enfermedad cardiovascular aterosclerótica o múltiples factores de riesgo) que tienen riesgo inflamatorio residual, siendo el primer agente antiinflamatorio aprobado como tratamiento ateroprotector cardiovascular.
Debemos recordar que la colchicina es un medicamento antiinflamatorio empleado en casos de gota. Pero un estudio publicado (ensayo COLCOT) hace unos años ya demostró que se asociaba a un menor riesgo de eventos CV isquémicos (23%) en pacientes con infarto de miocardio reciente. Otro estudio (LoDoCo2) mostró un menor riesgo de eventos CV (31%) en pacientes con enfermedad coronaria crónica.
Se trata de un fármaco seguro que debe excluirse en pacientes con enfermedad renal crónica y la enfermedad hepática crónica.
Un estudio reciente ha demostrado que entre los pacientes que reciben estatinas, la inflamación evaluada por PCR de alta sensibilidad fue un predictor más fuerte del riesgo de futuros eventos cardiovasculares y muerte que el colesterol evaluado por LDLC.
Estos datos tienen implicaciones para la selección de tratamientos complementarios más allá de la terapia con estatinas y sugieren que podría ser necesario el uso combinado de terapias agresivas para reducir los lípidos e inhibir la inflamación para reducir aún más el riesgo de aterosclerosis. Por ello la colchicina podría tener un papel mucho más importante que el que tiene actualmente entre nuestros pacientes cardiópatas. Veremos si realmente empezamos a emplearlo con estos hallazgos.
